  • All customers will need to complete a Par-Q screening form prior to attending any exercise class. If you have not already completed a Par-Q form, please ask at reception.
  • Upon completing the form, please hand to your class instructor to sign the form off, to enable you to participate. If you have any medical or health related problems that result in the instructor being unable to sign your form off, you will need to obtain a doctors note or letter to confirm that they are happy for you to attend the classes. In this instance, if a note cannot be obtained, you will not be permitted to participate in the classes.
  • Once you have completed a Par-Q it will be kept on file and used as a reference in the event of an emergency. If you have completed a Par-Q form and your health changes at any point, you must contact reception where you will be asked to complete a new form.
